By referring you to our office, your dentist has shown their concern that you receive the finest endodontic care possible. Our goal is to provide unsurpassed quality in a compassionate environment of professionalism and clinical excellence. We hope to exceed your expectations.
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Frequently, treatment can be initiated the same day as the consultation.
However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any X-rays if applicable. However, we will take X-rays before the evaluation and during treatment
- Photo identification
- A list of medications you are currently taking
- If you have medical or dental insurance, bring the necessary ID card and completed forms (This will save time and allow us to help you process any claims)
IMPORTANT: A parent or guardian must accompany all patients under 18
Please alert our office if you have a medical condition that may be of concern prior to surgery (uncontrolled systemic disease, artificial heart valves and joints or rheumatic fever etc), or if you require premedication prior to dental treatment (prophylactic antibiotics)
